BodyInc is a technology platform.

BodyInc:

• does not provide medical care

• does not diagnose, treat, or prescribe

• does not operate a pharmacy

• does not control medical decisions

3. No Medical Relationship

No provider-patient relationship exists between you and BodyInc.

All clinical relationships are between you and independent providers.

4. Independent Providers

Providers:

• operate independently

• use their own licensure and NPI

• make all clinical decisions

BodyInc does not supervise or control providers.

5. Provider Identity, Availability, and Non-Reliance

BodyInc does not guarantee the identity, availability, or continued participation of any provider.

You agree:

• no expectation of any specific provider

• no reliance on provider identity

• providers may be changed at any time

Provider changes:

• do not create liability

• do not create refund rights

6. No Continuity of Care

No guarantee of ongoing care or provider access.

7. Eligibility

You must:

• be 18+

• be located in the continental United States

8. Nature of Payments

Payments are for:

• platform access

• coordination services

Not guaranteed outcomes or products.

9. No Guarantees

No guarantee of:

• prescriptions

• treatment approval

• results

• availability

10. Consultations

Refunds ONLY if:

• provider fails to attend

• system failure attributable to BodyInc

NO refunds for:

• missed appointments

• failure to log in

• connectivity issues

• device issues

• user error

11. Connectivity Responsibility

User is responsible for:

• internet access

• device

• environment

Failure = missed appointment.

12. Prescriptions

All decisions made by independent providers.

13. Pharmacy

BodyInc does not control pharmacy operations or fulfillment.

14. Products

Non-refundable once shipped.

15. Subscriptions

• auto-renew

• user must cancel

• no retroactive refunds

• all charges final

16. Payment Authorization

User agrees:

• charges are authorized

• no disputes for valid charges

Improper disputes may result in enforcement actions.

18. Platform Control

BodyInc may suspend, deny, or terminate access at any time.

19. Intellectual Property

All systems and software are owned by BodyInc.

20. Disclaimer of Warranties

Platform provided “as is.”

21. Limitation of Liability

Liability limited to amount paid.

BodyInc not liable for:

• medical outcomes

• delays

• connectivity issues

• third-party actions

22. Indemnification

User agrees to defend and hold BodyInc harmless.

23. Arbitration

All disputes resolved via binding arbitration in Texas.

24. Class Action Waiver

No class actions permitted.

25. Jury Waiver

No jury trial.

26. Time Limitation

Claims must be brought within one (1) year.

27. Governing Law

Texas law applies.

28. Enforcement Rights

BodyInc may seek immediate legal relief for misuse.
